Underdog is pretty much a worse version of Pulsewave, it gives you a large boost to reload speed when you are critically wounded, but it doesn’t add the bonus handling like Pulsewave does. Although there are better trait perks, that doesn’t mean that Underdog is a bad perk, it can still be used to reload your weapon very fast in those tight scenarios where you would usually die if you didn’t have a reload boost.

Sadly, Underdog is too niche of a perk with too little of an impact. While up to +100 reload speed is nice when injured, most of the time it would be better off using a general reload speed perk like Outlaw, Rapid Hit, or Overflow. Even in a low health situation you would most likely be better off with Pulse Monitor, as it automatically loads a portion of your magazine and increases handling. This perk is one of the worst in the game and would need a serious buff to be considered as usable.

As of the Weapons team update of 05/15/24, Underdog seems to be completely shelved. All weapons that currently have Underdog on it will instead be changed to Pulse Monitor, making it the first perk to be entirely removed from the game, unlike Full Auto Trigger system.

Probably the worst of all "when critically wounded, you get a bonus" perks, already a rather contentious family of traits for their situational application. It needs very little explanation - when below half health, the weapon this perk is on will gain increased reload speed as you get closer to death, maximizing at (100 + 10%) reload speed when exactly 1 hit point away from death. While this perk is not useless - whether you know it or not, this perk *has* surely saved your life or at least helped you a few times unless you're really new - why people don't like it should be clear from just reading that sentence. Unlike other low-health traits, it only provides a single bonus (albeit to a decently important stat), and also unlike them, it does not have a set point before death where the effect maximizes. Pulse Monitor and Eye of the Storm reach maximum effect before you get to the absolute minimum possible health value (because there's no realistic situation in the game where you'll even be reduced to exactly 1HP, much less live very long even if you did), and on the topic of those, Pulse Monitor largely invalidates Underdog because, while it has a lower health activation threshold (shield break threshold, which is lower than half your health), it gives you an instant magazine refill on top of its other effects, which is more useful than "just" a reload speed bonus.

Only real synergy I see with this perk is with mask of the quiet one and protective light in pve. Let’s just say there’s better options and hopefully underdog and mask both get reworked/buffed hard because heart of inmost light+outlaw blows this out of the water 99% of the time.
